Die Fibromyalgie ist eine chronische Schmerzerkrankung, deren genaue Ursache noch nicht abschließend geklärt ist. Etwa vier Prozent der Bevölkerung sind betroffen, insbesondere Frauen im Alter von vierzig bis sechzig Jahren.

Inzwischen gibt es Hinweise darauf, dass bei den Betroffenen die Reizverarbeitung im Zentralnervensystem gestört sein könnte. Reize aus dem Innern des Körpers, die üblicherweise im Unbewussten verbleiben, werden von den Patienten bewusst wahrgenommen. Psychoanalytisch gesprochen: Die Verdrängungsschranke für bestimmte Körperreize ist herabgesetzt. So kommt es zu einer Art Hypersensibilität und Hyperaufmerksamkeit für Missempfindungen, die sich zu einem chronischen Schmerz- und Erschöpfungssyndrom steigern können. Die Beschwerden sind sehr vielfältig und äußern sich z.B. häufig in einem chronischen Faser-Muskel-Schmerz, der in der Regel großflächig in der Muskulatur um die Gelenke oder im Rücken auftritt.

Begleitende Beschwerden sind u.a. Müdigkeit, Schlafstörungen, Konzentrationsschwäche und Antriebsschwäche. Der nächtliche Schlaf wird von den Betroffenen meist nicht als erholsam empfunden. Psychische Beeinträchtigungen bei der Fibromyalgie sind sehr häufig depressive Störungen (Depression) und Angststörungen (Angst, Ängste) als Folge der Erkrankung.

Die Diagnose wird in der Regel vom behandelnden Facharzt für Neurologie gestellt.

Eine psychotherapeutische positive Beeinflussung der Fibromyalgie ist möglich, wenn die Betroffenen lernen, wie sie ihre Schmerzen durch weniger Stress bzw. Stressabbau und Abbau ihrer Hyperaufmerksamkeit günstig beeinflussen können.

Behandlungsmöglichkeiten

Eine Psychotherapie kann den Betroffenen dabei helfen, schmerzverstärkende bzw. schmerzlindernde Lebenssituationen und innere psychische Einstellungen zu den Beschwerden zu identifizieren und so dazu beitragen, die Schmerzen aktiv zu regulieren.

Durch Entspannung, innere Ruhe und Gelassenheit und ein Zurückführen der Missempfindungen in den Bereich des Unbewussten können sowohl die Schmerzen als auch die Schlafstörungen vermindert werden. Ein Abbau der Hypersensibilität gegenüber Empfindungen aus dem Innern des Körpers und sinnvoll eingesetzte Bewegungsübungen und Sport können dabei helfen, psychischen Stress und körperliche Verspannungen abzubauen.

Durch eine verbesserte Arbeitsorganisation ist es möglich, Stressfaktoren im Berufsleben zu reduzieren. Die Entspannung konflikthafter sozialer Beziehungen kann ebenfalls erheblich den psychischen Stress im Alltag verringern. Nicht zuletzt auch überzogene Ansprüche an sich selbst sollten kritisch als mögliche Stressfaktoren überprüft werden.

Eine analytische Behandlung ist mit Vorsicht in Erwägung zu ziehen, weil diese Form der Psychotherapie in erster Linie darauf abzielt, die Verdrängungsschranke herabzusetzen, um unbewussten Impulsen den Zugang zu bewussten Teilen der Psyche zu erleichtern. Sollte in Einzelfällen eine analytische Psychotherapie angewendet werden, ist es ratsam, sich an Parameter zu halten, die verhindern, dass sich ein kontraproduktiver Effekt ergibt.

Zusätzlich zur Psychotherapie sind als unterstützende Maßnahmen ein leichtes Ausdauertraining, leichte Kraftübungen, wohltuende Massagen und meditative Bewegungsübungen wie Qi-Gong, Tai-Chi oder Yoga sinnvoll (diese optionalen Anwendungen werden in meiner Praxis allerdings nicht angeboten).

In meiner Praxis kommt als Behandlungsform die tiefenpsychologisch fundierte Psychotherapie zur Anwendung. Zusätzlich können im Einzelfall auch Entspannungsverfahren wie autogenes Training oder handlungsorientierte Verfahren hinzugezogen werden.
